Hi Katie, how are you? I got your test results and wanted to go over them with you? Is that okay?

Yes, let's go over them and then I will explain how you can get access to your data.

So, I see here that your bloodwork shows some elevations in creatine, eGFR and some other kidney markers. Sometimes, when these numbers are elevated, it could suggest some early kidney disease. I would like to run more tests and send you for an ultrasound and set up a referral to a renal specialist. How does that sound?

Sometimes the blood tests indicate that something is going on in your body even if you're not experiencing any symptoms. These additional tests will help us to find out more. I will have my assistant email you all the requisitions. In order for me to send a referral to the specialist, I will need your permission. You will receive a document that will require your electronic signature in order for me to release this information to the specialist. With regards to your test results you will have to log into the lab portal to receive this information. You should have received a document from the lab where you had your tests done with log-in information. If you did not receive it, please contact them directly.

When you receive these emails, you will be prompted to verify yourself before you can access any of the requisitions. Once you have done so you will be able to print or store them on your phone to bring with you to the lab.

If you provide your consent, you can select individuals as part of your care team to see this information. The system will prompt you to add individuals and they will receive an email to confirm.

Yes, once you provide release of information, we will send the referral and share the necessary information and you will receive a follow-up from them.

Yes, they will send me a copy of any tests results and summary of your visit so we can coordinate your care.

I will have my assistant book a follow-up appointment once I receive your new test results.
